Duke University Health System-  Duke University Hospital  seeks to hire an experienced Transplant Coordinator who will embrace our mission of  Advancing Health Together.

 

Department Profile:


The Inpatient  Transplant Coordinator  at  Duke University Hospital  is active in delivering exemplary care for each patient to ensure excellent patient experience, every time. The transplant coordinator role has direct patient contact by attending daily transplant rounds with the transplant interdisciplinary team prepared to discuss clinical, psychosocial, or educational concerns regarding fresh pre and post-transplant patients for adult liver and kidney.

 

  • Educate patients and families regarding all aspects of post transplantation care, including but not limited to, arranging follow up appointments, labs, and working with the multidisciplinary team on patient specific needs such as outpatient pulmonary rehab, IV therapies, and medication management.
  • Communicate with patients and caregivers regarding the recovery process and facilitate discharge planning needs.
  • Compose written transplant education summaries to send post-transplant coordinator, provide patient or caregiver work letters, documenting education done in the Electronic Medical Record, the outcome of education.
  • Conduct post-discharge transplant calls 24-48 hours after discharge to review education, discuss future appointments, and address any outstanding concern or questions. 
  • Document inpatient summaries to send the post-transplant coordinator about care provided in hospital and changes made to the plan of care.
  • Ability to make clinical judgements based upon synthesis of current research, concepts, principles, theories, and outcomes to solve complex problems.
  • Collaborate with medical faculty, pharmacy, nursing personnel, case management and other health disciplines to facilitate meeting patient and family needs.
  • Staying current on post-transplant research and applying evidence based research and recommendations to patients/families on care of the transplant patient.
  • Complete regulatory documentation in UNOS data base / hospital EMR.
  • Reviewing education materials and working with department if there are changes in current practice.

 

This position does require on call responsibilities shared with a large team.

 

The transplant coordinator is an integral member of a larger interdisciplinary team contributing to excellence in patient care, research, teaching, and in providing leadership within the organization.

Work Hours and Shifts:

Full-time: 40 hour day shift, on call to include weekends/holiday rotational coverage

Education
BSN required, MSN preferred.


Experience  5 years nursing experience with at least two years in transplant, critical care, or chronic disease specific setting.


Degrees, Licensures, Certifications  Current or compact RN licensure in the state of North Carolina required. BLS required.
