Job Description:
- Provides direct psychiatric services to the ACT Team clients and adults in the Medical Services Program as needed at multiple District 19 clinics.
- Conducts comprehensive psychiatric assessments and evaluations to determine appropriate diagnosis and course of treatment.
- Makes referrals and recommendations to additional providers as appropriate.
- Provides or assures that all consumers receive appropriate medical & primary care screening, psychiatric evaluation and diagnosis, and that all medical/psychiatric care is appropriately & timely delivered.
- Submits prescriptions electronically, accurately and timely for patients.
- Follows all established agency policies and procedures.
- Completes comprehensive psychiatric evaluations for ACT program admissions.
- Contact with ACT program consumers at a minimum, on a quarterly basis.
- Participates in team meetings, treatment planning, recommendations to improve psychiatric engagement, and assessment of medical and psychiatric needs.
- Establishes and modifies treatment plans including diagnosis as needed.
- Monitors client's response to treatment including medication side effects and adverse reactions.
- Orders and interprets lab results.
- Demonstrates excellent customer service, professional communication, and teamwork.
- Participates in administrative and clinical staff meetings as required.
- Provides consultation to agency staff about clinical and medical services.
- Collaborates with interdisciplinary team members and other medical providers, families and community providers.
- Communicates assessment information, treatment planning, interventions and crisis need with Case Managers, clinicians, supervisors and stakeholders.
- Promotes integration of care through consultation and collaborative efforts.
- Participates in meetings and conferences as directed by supervisor.
- Responds promptly and professionally to e-mail inquiries, voicemail messages, and other necessary communications.
- Staying current with new medications and best practice through research, training, and education. Prescribing the latest in new psychotropic medication as appropriate.
- Provide consultation and guidance for Primary Care Screening and Monitoring program.
- Coordinates special projects as needed to support medical and psychiatric service provision.
- Initiates health promotion initiatives.
- Documents and signs all services provided in consumer record on the same day that service is provided.
- Records all services and needed documentation correctly in D19 CSB's EHR with correct codes and services within 24 hours of service delivery.
- Complies with all established legislation, mandate programs standards, and all District 19 Clinical Services requirements regarding consumer services and documentation of service delivery.
- Adheres to daily patient schedule to include emergency visits, post hospitalization follow up visits, and other acute needs.
- Provides psychiatric involvement in the development and review of policies, procedures and protocol that govern care.
- Makes recommendations for continuous quality improvement for the ACT Team and Medical Services.
- Functions as a change agent to identify and implement process improvement initiatives.
- Must stay abreast of changes in laws, mandates, and regulations that impact practice and care delivery and recommend modifications to remain in compliance.
- Follows up within 2 business days of being notified of needed revisions from chart audits.
- Completes all required trainings per agency and regulatory guidelines.
